The ADAPTE practice guideline adaptation framework evaluation Study |
- The ADAPTE framework provides a systematic approach for the adaptation of guidelines produced in one setting to be used in a different cultural and organizational context.
- Although the ADAPTE framework has been thoroughly developed and tested, it is important to evaluate its merits and limitations before distributing it to the population at large. Subsequently, the ADAPTE Manual and Resource toolkit are launched together with an evaluation study.
- During the evaluation study period, potential users of the ADAPTE Manual and Resource toolkit will be systematically asked to register and participate in the evaluation study.
The ADAPTE Evaluation Study
Participation in the evaluation study will allow the user to obtain the ADAPTE Manual and Resource toolkit, and to receive support during the adaptation process.
ADAPTE requests that participants of the evaluation study follow these steps:
- fill in registration form (7 questions and contact details) and consent form
- obtain ADAPTE Manual
- read and evaluate ADAPTE Manual
- filling in first perception form (8 questions)
- obtain ADAPTE Resource Toolkit
- read and evaluate ADAPTE Resource toolkit
- fill in second perception form (5 questions)
- plan own adaptation process
- fill in guideline adaptation process form (16 questions)
- send own adaptation plan / protocol to ADAPTE
- conduct guideline adaptation process
- assess usefulness, clarity, comprehensiveness and feasibility of the ADAPTE adaptation process
- report on elements on final evaluation form (37 questions, comments)
- send copies of the guideline developed using the ADAPTE framework
According to modified French Law on Informatics, Files and Liberties 78-17 of January 6th, 1978, this website has been declared to the Commission Nationale de l’Informatique et des Libertés (CNIL).
Registrations and gathering of data are in agreement with the rules edited by the CNIL. Data provided by users will not be used commercially but solely by the ADAPTE Collaboration within its evaluation study.

Evaluation of the ADAPTE practice guideline adaptation framework |
Background
Developing and updating high quality CPGs require substantial time, expertise and resources.
Though key methods for guidelines development have converged over the years, most worldwide organisations producing CPGs face increasing pressure to produce more CPGs, more rapidly, with fewer resources. In addition, one of the crucial aspects of using CPGs to improve knowledge transfer and uptake is their implementation in practice. Insufficient attention to the implementation phase and a lack of highly effective methods have hampered the effectiveness of CPGs to change practice significantly.
Therefore, there has been a growing interest in developing a valid methodology to adapt existing high quality guidelines instead of developing new ones entirely de novo. Such an adaptation process may also help improve the effective implementation of CPGs. Based on preliminary work and a literature review, the international ADAPTE Collaboration has developed a stepwise approach to the adaptation of CPGs which includes three phases, nine modules, and 24 steps described and illustrated in a manual and supported by a resource toolkit.
Working hypothesis
Adapting well developed existing CPGs using the ADAPTE process, manual, resource toolkit and support allows a feasible and efficient process to be conducted as well as to obtain high quality guidelines more rapidly and using less resources. This ‘utilization-focused’, formative, evaluation will allow improving the ADAPTE framework.
Specific aims
The specific aims of the Evaluation Study are:
- to determine how the ADAPTE process, the support manual and tools are actually used in practice;
- to examine the acceptance and practicability of the ADAPTE process, manual and tools and user satisfaction;
- to assess the quality and content of adapted guidelines resulting from using the ADAPTE process;
- to document the resources used when adapting guidelines by means of the ADAPTE process and to estimate the possible savings comparing with de novo guideline development;
- to modify and refine the ADAPTE process according to the results of the evaluation study.
Methods
Based on a conceptual framework, we have developed relevant research questions corresponding to our objectives and translated them into forms (closed Likert scale and open questions) to be filled in by participants with various degrees of experience (guidelines agencies and novices) and from various locations worldwide. The ADAPTE framework will undergo several phases of evaluation; before (profile and initial reactions to the ADAPTE framework), during (advantages, barriers and difficulties of every phase, module and step) and post-use (overall experience, satisfaction, quality of adapted CPG, time and resources used). Forms will be filled-in using a web-based approach. As CPG adaptation teams may not have the same experiences using the process, we will attempt to capture these different perspectives by eliciting comments and feedback on the survey. The output of the evaluation will be based on a quantified descriptive summary and a qualitative synthesis of the experience of various CPG adaptation processes.
Expected value of the proposed project
The evaluation of the ADAPTE framework in various settings will produce new knowledge about the value and potential advantages in process, time and resources of using a stepwise approach to adapt existing guidelines. The results will allow improvement of the ADAPTE framework, manual and resource toolkit for guideline adaptation, and will contribute to achieve the aims of ADAPTE i.e. enhancing the efficiency in guideline development and improving utilisation.
